Using Your Plan
Application step-by-step
Step 1 – select a plan from the Rates & Plan Summaries Section above
Step 2 – print out the Delta Dental or Delta Care application from the Forms Library below and complete (to find a Delta Dental Dentist please click here)
Step 3 – PAYMENT METHOD
Print out Dental / Vision Payment Worksheet from the Forms Library below to determine final monthly payment.
There are two options for payment:
- Auto deduction directly from your checking account –
Fill out the Kelsey Auto Deduction Form from the Forms Library below and attach a voided check
- Receive a monthly bill and mail in payment
Prepare a check for the 1st months premium made payable to Kelsey National (NOT VANTAGE !!!)
(Please be aware that if you choose the monthly billing method and your monthly premium payment does not reach the processing center by the due date your policy may be cancelled.)
Step 4 – CCAE MEMBERSHIP
You must be a member of CCAE (California Council for Adult Educators) in order to qualify for membership benefits.
If you are already a member, please proceed to Step 5.
If you are not a member please either:
- Print out and complete the membership form from the Forms Library below
- If you have questions you can contact CCAE membership at membership@ccaestate.org or call them at 888.542.2131
CCAE has a variety of membership plans available.
Step 5 – FINAL CHECKLIST AND MAILING
Put the following documents in an envelope and mail to:

- Completed Delta Dental or Delta Care application
- Dental / Vision Payment Worksheet
- Copy of Child Care License and Drivers license
- Copy of CCAE card or application for membership
- Auto Pay Form and VOID check or 1st months payment
