Application step-by-step

Step 1 – select a plan from the Rates & Plan Summaries Section above

Step 2 – print out the Western Health application from the Forms Library below and complete.
(to find a Western Health Doctor or Hospital please click here)

Step 3 – JOINER FORMS
print out and complete the CAFCC Joiner Forms from the Forms Library below.

Step 4 – PAYMENT METHOD
There are two options for payment:

  • Auto deduction directly from your checking account –
    Fill out the BAIS Auto Deduction Form from the Forms Library below and attach a voided check
  • Receive a monthly bill and mail in payment
    Prepare a check for the 1st months premium made payable to BA&IS (NOT VANTAGE!)

(Please be aware that if you choose the monthly billing method and your monthly premium payment does not reach the processing center by the due date your policy may be cancelled.)

Step 5 – CAFCC MEMBERSHIP

You must be a member of CAFCC (California Association for Family Child Care) in order to qualify for membership benefits.

If you are already a member, please proceed to Step 6.

If you are not a member please either:

  • Print out and complete the membership form from the Forms Library below
  • Click on the link to fill out an online application

CAFCC has a variety of membership plans in addition to discounts for members of a local Child Care Association or the National Association (NAFCC).

Applications are in both English and Spanish.

Step 6 – FINAL CHECKLIST AND MAILING   

Put the following documents in an envelope and mail to:

  1. Completed Western Health application
  2. Both Joiner Forms
  3. Copy of Child Care License and Drivers license
  4. Copy of CAFCC card or application for membership
  5. Auto Pay Form and VOID check or 1st months payment 

Step 7 – Refer an Association member or friend

Don’t forget, that if you refer an Association Member or a friend for ANY of the products Vantage has to offer, and they become a client of Vantage, we will send you a $25 gift card to the establishment of your choice. Please click on the “Member Referral Form” from the Forms Library below. Complete it. Send it to us.

Whether you visit your doctor regularly or rarely, building a relationship with a physician that you select is an important part of your health care. Each member of your family should have his/her own physician for preventive care, treatment of illness, and referral to a specialist when needed. Whether you're a new or long-time member, we encourage you to take this vital step and select a personal physician.

WHA's Provider and Hospital Network

Among WHA's greatest strengths are our top-quality providers and owner-sponsor groups. They are among the best in northern California.

Mercy Healthcare Sacramento is a division of Catholic Healthcare West Medical Foundation, a California non-profit benefit corporation. Mercy Medical Group provides health care services to Sacramento area families through its contracted multi-specialty group of more than 200 practitioners. MMG is an affiliate of Catholic Healthcare West and CHW's Mercy hospitals in Sacramento (including Mercy General Hospital, Mercy San Juan Medical Center, Mercy Hospital of Folsom, and Methodist Hospital of Sacramento).

UC Davis Health System represents one of WHA's major providers for primary, tertiary, and trauma services. The system encompasses the UCD School of Medicine's clinical faculty, health care professionals of the medical center and a geographically dispersed network of primary care providers.

NorthBay Healthcare is a non-profit health care organization serving Solano County since 1960. Their hospitals which include NorthBay Medical Center in Fairfield and NorthBay VacaValley Hospital in Vacaville, are known for their advanced, compassionate care. The NorthBay Center for Primary Care, with offices in Fairfield, Vacaville and Green Valley, is a partnership between NorthBay Healthcare and a group of respected local physicians.

Woodland Healthcare is comprised of Woodland Memorial Hospital, Woodland Clinic Medical Group, and Woodland Healthcare Davis, and offers a wide range of medical services. Woodland Healthcare is one of Northern California's first truly integrated inpatient and outpatient health care delivery systems.

Hill Physicians Medical Group provides healthcare services to approximately 370,000 patient members in Northern California with a network of almost 2400 physicians and other healthcare providers. Formed in 1984, HPMG is one of the largest independent practice associations (IPA).

Golden State Physicians Medical Group is a group of 500 independent physicians and health care providers, including 124 primary care physicians and more than 350 specialists, providing services in more than 38 specialties.

If you are looking for a class to join, WHA's Health Resources Directory provides a list of classes sponsored through WHA's medical groups. As a WHA member, you can sign up for class regardless of your group affiliation.

Welcome to WHA's Health Promotion webpage. Our health promotion services combine education and intervention to create awareness about choices that encourage a healthy lifestyle. On this page, you'll be able to view print-friendly and downloadable guidelines for you and your family. The guidelines were developed based on recommendations from nationally recognized sources such as the United States Preventive Services Task Force (USPSTF), American Academy of Pediatrics, U.S. Department of Health & Human Services, American College of Obstetricians & Gynecologist, and the Center for Disease Control & Prevention.

In addition, you'll be able to access wellness tools where you can complete a Personal Health Assessment and design a program that supports your optimal health goals. You'll also learn about our incentive programs and health club discount offers.

Nurse Advice Line

Clinical nurse support is now available 24 hours a day, seven days a week. The service is offered through Alere® and is staffed by highly-trained, registered nurses who can help answer questions about a medical problem you may have including:

  • Caring for minor injuries and illnesses
  • Seeking the most appropriate help based on the medical concern
  • Identifying and addressing emergency medical concerns
  • Preparing for doctor visits
  • Understanding prescription medications
  • Helping with lifestyle choices to improve health
  • Providing education and support regarding decisions about tests

If an urgent situation is discovered during a call, nurses can guide you to the most appropriate course of action. Guidance can include referrals to hospital emergency rooms, urgent care centers, 911 or your physician. You can access the Nurse Advice Line services by calling this toll-free number: (877)793-3655
