Application step-by-step

Step 1 – select a plan from the Rates & Plan Summaries Section above

Step 2 – print out the Care 1st application from the Forms Library below and complete.
(to find a Care 1st Doctor or Hospital please click here)

Step 3 – JOINER FORMS
print out and complete the CEA Joiner Forms from the Forms Library below.

Step 4 – Health Questionnaire
Please print out the Health Questionnaire from the Forms Library below.

Step 5 – PAYMENT METHOD
There are two options for payment:

  • Auto deduction directly from your checking account –
    Fill out the BAIS Auto Deduction Form from the Forms Library below and attach a voided check
  • Receive a monthly bill and mail in payment
    Prepare a check for the 1st months premium made payable to BA&IS (NOT VANTAGE!)

(Please be aware that if you choose the monthly billing method and your monthly premium payment does not reach the processing center by the due date your policy may be cancelled.)

Step 6 – CAFCC MEMBERSHIP

You must be a member of CAFCC (California Association for Family Child Care) in order to qualify for membership benefits.

If you are already a member, please proceed to Step 7.

If you are not a member please either:

  • Print out and complete the membership form from the Forms Library below
  • Click on the link to fill out an online application

CAFCC has a variety of membership plans in addition to discounts for members of a local Child Care Association or the National Association (NAFCC).

Applications are in both English and Spanish.

Step 7 – FINAL CHECKLIST AND MAILING   

Put the following documents in an envelope and mail to:

  1. Completed Care 1st application
  2. Health Questionnaire
  3. Both Joiner Forms
  4. Copy of Child Care License and Drivers license
  5. Copy of CAFCC card or application for membership
  6. Auto Pay Form and VOID check or 1st months payment

Step 8 – Refer an Association member or friend

Don’t forget, that if you refer an Association Member or a friend for ANY of the products Vantage has to offer, and they become a client of Vantage, we will send you a $25 gift card to the establishment of your choice. Please click on the “Member Referral Form” from the Forms Library below. Complete it. Send it to us.

Health Education For Members

Care1st Health Education offers many family health services. We want to help you and your family to stay healthy and active, so Care1st provides services, materials and information on various related topics. If you would like more information on the services available to you, please call the Health Education Department at 1-800-605-2556.

Services

Health Education provides information to help you stay fit, manage your disease, or maintain your weight, eat healthy, and many more topics. We can help you find classes, counseling and support groups for your needs.

We offer a Living Well information line to answer your questions on cholesterol, weight management, exercise, general nutrition guidelines, and diabetes. To access our Living Well information line please call the Health Education Department at 1-800-605-2556.

Preventive Health (Initial Health Assessment - First Health Checkup)

You need to make an appointment for your first health checkup as soon as possible if you are a Care1st member. You will receive a complete physical during your first health checkup. This checkup will help your doctor get to know you better. This is important since your doctor is your partner in your health. At your appointment, your doctor will ask you to answer a questionnaire called the "Staying Healthy Assessment." It will help your doctor provide you with complete care.

We want you and your family to stay healthy. Regular checkups, even when you are not sick, are important to maintain your health. They also help your doctor identify health problems early on. Preventive health guidelines below show you when you need your regular checkups:

Your doctor's office may have brochures on various health topics for you and your family. Or you may also request brochures by calling Care1st's Health Education Department:

Member Newsletters

As a Care1st member, you will receive newsletters that have useful information on many topics. An online version of the most current newsletter may be viewed (English & Spanish).
